|Job Description:||We seek an experienced, compassionate and dedicated individual to lead TOUCH’s medical case management team (MCM), including our Ryan White Act MCM program for people with HIV. The MCM Program Manager will also serve as a case manager for some TOUCH clients and help coordinate MCM client services with TOUCH’s food pantry and nutrition programs. The MCM Program Manager will report to the Executive Director. The primary responsibility of this position is to ensure that TOUCH’s MCM programs provide excellent services to our members/clients (to help them achieve their optimal health and well-being, including their medical and nutritional needs. Toward this goal, the MCM Program Manager’s job responsibilities will include the following: 1. Day to day participation in and supervision of TOUCH’s case management team as they provide the following services: a) Ensuring client eligibility; b) intake assessments; c) service plan development and implementation; d) linkages and referrals to needed services; e) home visits and accompaniment to services when needed; f) promoting health education for clients; and g) outreach for case findings and client re-engagement 2. Ensuring that TOUCH is fulfilling its contractual MCM program obligations, including meeting service goals, monthly and annual reporting requirements and HIPAA compliance; 3. Ability to use online reporting systems (e.g., eSHARE or similar systems) to document services; ensure accurate record keeping; and generate reports 4. Completing annual professional development requirements and certifications 5. Networking with referral sources 6. Other duties as assigned by the Executive Director|
|Requirements:||· Commitment to excellence and to contributing to a positive and collaborative workplace · A minimum of 3 years of relevant professional experience with a track record as a highly effective manager, supervisor and team builder with strong planning and organizational skills · College degree in human services or equivalent experience, MSW or other relevant Master’s degree preferred · A demonstrated knowledge of health issues, including nutrition, food insecurity, HIV care and other chronic diseases · High level of English proficiency, Spanish or Creole language skills a big plus · Budgeting and fiscal management skills Driver’s license and access to a personal vehicle for home visits when needed. Mileage is reimbursed|

|Salary:||$55,000-$65,000 based on education and experience|
