Job Description:Manage assigned cases in the Supported Housing Program. MAIN RESPONSIBILITIES: • Develop and promote appropriate services for all clients/tenants. Provide services or connect tenants to job training/placement, legal services, budgeting, entitlements, behavioral of physical health services, eviction prevention programs to support tenants in maintaining their housing. Provides recipients with information about support services and assistance. • Responsible for communicating with outside community agencies and other departments regarding individuals. • Responsible for overseeing documentation of individuals; i.e., all documentation is properly filled out and properly recorded in the individual case records. Ensures clinical records and related source documents are maintained in accordance with industry and agency requirements. • Ensure that tenants are aware of tenancy obligations through tenant orientation, tenant handbook, and individual meetings with tenants • Identify households in distress and works in collaboration with property coordination to prevent lease compliance issues and eviction. Meet regularly with property coordination staff to review tenants in arrears and implement eviction prevention programs including rent payment plans. • Work independently with some supervision. • Visit each consumer a minimum of twice per month, once a week as necessary. Qualifications:BA degree in a Human Services field. One (1) year of work experience with Mental Health population.
Skills:Must have a valid, non-restricted driver's license. Bilingual (English/Spanish) preferred but not required.
Certifications:BA degree in Human Services
Hourly Wage:$24.00-$27.47
