POSITION SUMMARY:Community Habilitation Provider provides skill development training for a supported individual in and outside of individual’s home. Community Habilitation Provider must work effectively with the individual receiving services as well as family members and must provide a safe vehicle for transporting person receiving services during sessions (unless hired for a non-driving position).
POSITION QUALIFICATIONS:Minimum Education: High School Diploma or equivalent required; college a plus. Minimum Experience: Experience working with people with intellectual/developmental disabilities preferred.
Knowledge/Skills/Abilities(KSAs):Required Certification/Registration: N/A
Physical Requirements:Must have visual acuity adequate to read correspondence, computer screen, forms, etc.; manual dexterity; speaking and hearing ability sufficient to communicate by phone or in person at normal volumes, etc. Must be able to drive and maintain a valid NYS driver’s license (unless hired or a non-driving position). Must be able to walk, stand, bend, and stoop.
Working Conditions:Work involves regular exposure with supported individuals, and may take place in a private home or a community setting.
Corporate Compliance:Employees must comply with agency or governing body rules, regulations and policies and procedures. Any employee who witnesses or learns about a violation or possible violation of agency or governing body rules, regulations or policies and procedures is required to immediately report this to his/her supervisor or the agency’s corporate compliance officer

ESSENTIAL FUNCTIONS:Train individual receiving services to develop/increase independence with verbal prompts, modeling and hand-over-hand assistance, as needed. Document training accurately and contemporaneously. Turn in billing and all documentation by Tuesday following each payday. Turn in Monthly Notes by the 15th of each month, for the previous month. Learn and utilize appropriate safeguards to ensure the supported individual’s safety and well-being. Communicate progress and concerns with family and supervisors regularly.
