| Job Description: | "Appropriately assist clients with personal care activities and provide companionship tasks in accordance with the care plan. Appropriately assist with household tasks directly related to the client’s health and in accordance with the care plan. Accurately measure and record temperatures, pulse, respiration and blood pressure. Utilize equipment in an appropriate and safe manner. Perform simple procedures as an extension of therapy services. Remind client for self-administration of medication. Document services provided accurately. Report any changes in the client’s condition or incidents immediately.
