We operate programs in the New York Metropolitan area and Hudson Valley. Job Summary: This position is located in Irvington, NY. Intake & Aftercare Coordinator (I & A Coordinator) The Intake and Aftercare Coordinator is responsible for the coordination of all intakes, discharge and aftercare services needed for each youth entering or departing an Abbott House QRTP Group Home. Intake The QRTP Intake & Aftercare Coordinator (I & A Coordinator) is responsible for the assessment and review of all referrals to the Abbot House QRTP Group Homes. The I & A Coordinator will closely partner with the Group Home Program and Administrative Team to determine appropriate candidates for admission. In addition, he/she will ensure that youth determined appropriate for the QRTP level of care are involved active participants in the admission process to ensure for a successful transition into the home. The I & A coordinator will lead all communications with regulatory and referral sources regarding potential admissions and will be the primary planner for video conference meet and greets and in person visitation to the homes. The I & A coordinator will also help to develop and produce an individual trauma history for every resident who is being admitted and will help to look at all admissions with a trauma informed lens. Aftercare & Discharge- Goals related to discharge and aftercare must be developed and presented in the first support plan and in every subsequent support plan thereafter while in the QRTP placement. The I & A coordinator will develop objectives that are measurable and may change throughout the youth’s placement at the Group Home to reflect progress toward achieving the goals. The I & A Coordinator will provide the required QRTP services for all Group Home youth for a minimum of six months post-discharge or step-down. Duties & Responsibilities: Expectations for in-facility discharge and aftercare planning include the following: Creation of the youth’s discharge plan in conjunction with the youth’s permanency team Face-to-face meetings with youth twice per month (one must include the family/discharge resource. Assessment of the needs of the youth’s family and referrals to community resources as needed. Facilitation of permanency/support team meetings no less than monthly with key members of the agency staff, the youth, the youth’s family, and other community resource members. Qualification:"About You You are the ideal candidate if you are passionate about the social services field and empowering underserved communities. Being compassionate, organized, self-motivated, driven, flexible and responsive are critical attributes to be successful in this role. You have a bachelor's degree, Master's degree preferred. You have 3 years of experience in Group Homes/Foster Care settings with at least 1 year in residential services. You have a valid driver's license. You have good written and oral communication skills. The ideal candidate will possess excellent computer skills to navigate and coordinate all meet and greets and gather comprehensive pertinent information from referral sources and stakeholders. The individual will have excellent organizational and communication skills and a deep commitment to ensuring that all potential candidates and families are treated with respect and actively empowered in the admission process."
