POSITION SUMMARY:Under the direction of the Community Supervisor for the Residence in conjunction with the Director of Nursing or Designee, the incumbent will provide hands-on direct care and nursing care by planning, organizing, documenting and administering care provided to individuals who are supported by The Arc Rockland. Care delivery will be done in compliance with rules and regulations of OPWDD and The Arc Rockland within the scope of the practice and assessed as such. The Licensed Practical Nurse will report to the Community Supervisor for the Residence, or Designee, for direct care and scheduling functions, and to the Director of Nursing, or Designee, on issues relating to nursing care and may assist in all care provided to the individuals at the assigned Residence.
ESSENTIAL FUNCTIONS:• Participate in Medication Management, including, but not limited to, administration of medication, maintenance of MAR’s, timely ordering of medications, the integrity and orderliness of the medication cart/room, and the appropriate disposition of outdated medications as per policy. • Maintain documentation as appropriate, including, but not limited to, accurate tickler files, medical records, appointment schedules and nurses’ notes. • Provide feedback and information and share concerns regarding supported individuals with the Community Supervisor and Director of Nursing or Designee. • Oversee and coordinate medical and dental appointments. May accompany individuals at discretion of Community Supervisor and Director of Nursing or Designee. • Participate in the formation of the Nursing Care Plan for each supported individual. • Participate in health training for supported individuals, including, but not limited to, recognizing signs and symptoms of medication side effects, disease process and management. • Monitor vital signs and response to medication and treatments. • Provide individual care as directed based on each individual’s Plan of Nursing Care. • Participate as directed in identification and prevention of abuse of supported individuals and identification of or provision of care in emergency situations. • Participate in health counseling based on the cognitive level of the individual – e.g., the importance of handwashing in infection control. • Provide nursing treatments relative to restorative care and health maintenance, including, but not limited to monitoring bowel and bladder training, tube feeding and wound/stoma care. • Monitor chronically ill individuals and report signs and symptoms of complications. • Participate in supportive care, including, but not limited to, chronic pain management, hospice and palliative care. • Ensure that all individuals’ medical records are in compliance with all applicable state regulations and requirements of The Arc Rockland. • Ensure that all supplies (medication, nursing forms, etc.) are available and current. • Support the interdisciplinary approach to individual care as well as team process and participate as directed by nursing supervisor. • Participate as a team member of the interdisciplinary care team. • May assist in the maintenance of the physical environment of the Residence, provide hands on assistance at mealtimes, and participate in recreational or social activities. • Performs all aspects of direct care for individuals, as directed by Community Supervisor or Designee.
ADDITIONAL RESPONSIBILITIES:• Maintain personal certifications (LPN, CPR, SCIP, etc.) • Support therapeutic and behavioral plans in partnership with clinical staff to enhance the quality of life for supported individuals. • Attend monthly staff meetings and other related meetings. • Summon police, fire, ambulance or other emergency personnel if situation warrants. • Attend and participate in required staff training programs. • Supervise other health-related activities to maintain program quality. • Assist in all health-related care provided to the Residence. • Performs other related duties as requested by the Community Supervisor, Director of Nursing or either’s Designee.
POSITION QUALIFICATIONS:• Minimum Education: Graduation from an accredited practical nursing program. • Licensing/Certification: Active New York State LPN License. Current BLS certification. SCIP certification must be obtained and maintained. • Minimum Experience: One year of experience working with developmentally disabled individuals as a Licensed Practical Nurse preferred. • Knowledge/Skills/Abilities (KSAs): · Ability to read, writes, speak and understand English proficiently. · Good oral and written communication skills. · Good organization skills. · Strong interpersonal skills. · Ability to prioritize assignments. · Must be able to maintain the strictest confidentiality and assure HIPAA compliance. · Proficiency in the use of a personal computer and appropriate software. · Understands and adheres to The Arc Rockland’s Code of Conduct, as outlined in the Employee Handbook and all policies and applicable procedures. Valid driver’s license; meets Agency’s driving standards as identified.
