| Job Description: | "Abbott House is seeking an energetic case planner to join the Preventive Case Planner team. In this role, you will be responsible for delivering casework services to at-risk families. The Preventive Case Planner is responsible for the delivery of case work services to at-risk families that have indicated reports of Child Abuse and/or neglect. Case Planners will help families develop the skills and resources needed to strengthen the family unit and prevent out of home and/or foster care placements. Case Planners will use a strengths-based, intensive case management approach to service delivery that includes providing direct supervision and counseling to the children and families, linking family members to appropriate community-based services, forming, and maintaining positive family relationships. | Qualification: | "Master’s Degree in Social Work or in a related field preferred. Minimum Bachelor’s in Social Work or in a related field and at least 3 years of experience required. Minimum of two years of case management experience. · Valid Driver’s License" |
| Skills: | Excellent counseling, communication, interpersonal and writing skills a must. Knowledge of community agencies within Dutchess County required. Must have excellent writing and communication skills and must be computer literate. |
