|Job Summary:||Responsible for the supervision of consumers in a community residence program serving Mentally Ill and/or Mentally Ill and Mentally Challenged individuals. Responsible for the safety of consumers, cleanliness of the residence, as well as providing and documenting restorative services based on accurate and consistent treatment plans which are reviewed quarterly. Attend staff development training. Conducts half hour bed checks or as needed. Sleeping on the job is prohibited.|

|Duties & Responsibilities:||- Carries out the goals, objectives, and the Mission of the Agency. - Is familiar with and follows policies and procedures outlined in the Agency Personnel Manual and the OMH Services Procedure Manual. - Contributes to the safety and therapeutic environment of the programs. - Portrays a positive role model for the consumers and appropriately advocates for their needs. - Attends and participates in weekly supervision session and staff meetings as requested by residence manager. - Provides supervision and training to consumers in the following areas: Personal healthcare and hygiene, e.g., bathing, washing, dental care personal safety, including assessment of self-help, fire drills and safety regarding the use of kitchen appliances. - Individual clothing care, personal belongings, bedroom and living quarters. - Food and meal planning, preparation, purchase and storage. - Use of household appliances, washer, dryer, refrigerator, freezer, vacuum cleaner and other small appliances. - Use of public transportation. Assists with and/or assumes the responsibility for cooking, cleaning, and cleaning up after a meal. - Supervises consumers while taking their medications, receive, log in and monitor intake of medications, assess for adverse reactions, count meds with the consumers, encourage consumers to take meds at the appropriate times. - Prepares service plans with the consumer, designed to improve the consumer’s abilities and overall functioning. - Assist in appropriate socialization. - Assist with job seeking skills Responsible for writing and maintaining appropriate documentation on assigned consumers, including progress notes, restorative services, goals and objectives, special logs and incident reports. Communicates effectively with members of other treatment programs, family, therapist, psychiatrist, and other community-based treatment programs. - Responsible for timely and accurate submission of time sheet. - Acts as a therapeutic change agent for those consumers presently in the agency and for the potential consumers applying for admission to the program. - Drive consumers to appointments when necessary. - Uses therapeutic and respectful strategies when dealing with consumers and staff. - Attends mandatory training as well as implementing individual annual staff training plan.|
|JOB SPECIFICATIONS (Skills, Knowledge, Ability):||CULTURAL AND SOCIOECONOMIC CHARACTERISTICS: Ability to interact with and complete assignments in a manner sensitive to the service population’s cultural and socioeconomic characteristics. - LANGUAGE SKILLS: Ability to read and comprehend simple instructions, short correspondence, and memos. Ability to write simple correspondence. Ability to effectively present information in one-on-one and small group situations to peers, consumers, families and other employees of the agency. - MATHEMATICAL SKILLS: Ability to add and subtract two digit numbers and to multiply and divide with 10’s and 100’s. Ability to perform these operations using units of American money and weight measurement. - REASONING ABILITY: Ability to apply common sense understanding to carry out detailed but uninvolved written or oral instructions. Ability to deal with problems involving a few concrete variables in standardized situations.|
|EDUCATION & EXPERIENCE:||High school graduate. Experience in a residence dealing with Mentally Ill and/or Mentally Ill and Mentally Challenged individuals required. BA with experience preferred. Valid New York State driver’s license.|
|Physical Requirements:||While performing the duties of this job, the employee frequently is required to stand or sit for long periods. The employee is occasionally required to sit and stoop, kneel, crouch, or crawl. The employee must occasionally lift and/or move up to 25 pounds.|
